The following must be included with completed registration form
(INCOMPLETE REGISTRATION FORMS WILL NOT BE PROCESSED)

• Cheque for Registration Fee post-dated no later than December 31, 2009
  (IF DATED LATER than December 31, 2009, REGISTRATION IS SUBJECT TO ADDITIONAL FEE OF $25)
• Post-dated (June 30, 2010) uniform deposit of $50 will be collected by team manager when uniforms are distributed)
• Copy of Birth Certificate for all players & proof of residency within the Langley Baseball Boundaries.
  
  
• Forms {along with payment (cheques only)} can be dropped of at Prostock Athletic,
